Good Faith Estimate of Cash Charges

Cash Pay: $85 per visit  (30 - 40 min.)

Run Gait Analysis: $120 per visit (full hour)

Dry Needling $30 per visit (covered under some insurances)

Laser Packages

6   Treatments          $240

9   Treatments          $315

12   Treatments        $360

I understand that I am responsible for the above cash pay charges should I not use insurance to cover my visit.  Dry needling and laser charges are normally not covered under insurance, the exception is that VA covers dry needling.  I understand that I will be responsible for these fees in addition to my insurance co-pays if my insurance does not cover these services.
